Waiver & Release of Liability

I, the undersigned, am the parent or legal guardian of [Your Child's Full Name] (hereafter referred to as "my child"). In consideration of my child being permitted to participate in LingoStars programs (the "Program"), I agree to the following terms and conditions:

  1. Voluntary Participation

I understand and acknowledge that my child's participation in the Program is entirely voluntary. I am aware that the activities involved in the Program may include, but are not limited to, physical activities, outdoor play, arts and crafts, and field trips. I assume all risks related to my child's participation.

  1. Assumption of Risk

I acknowledge that certain risks of injury, both minor and serious, exist while my child is participating in Program activities. I assume full responsibility for any injury, illness, or harm that my child may sustain as a result of their participation in the Program, including risks arising from the negligence of LingoStars, its staff, volunteers, or other participants.

  1. Medical Authorization

In the event of an emergency, I hereby authorize the Program staff or volunteers to take all necessary steps to secure medical treatment for my child. This includes calling for emergency medical personnel or transporting my child to the nearest hospital or medical facility. I further authorize medical treatment deemed necessary by a licensed physician or healthcare provider for the health and well-being of my child. I agree to be responsible for all medical costs incurred as a result of such treatment.

  1. Release of Liability

I hereby release, discharge, and hold harmless LingoStars its staff, volunteers, representatives, and agents from any and all liability, claims, demands, or causes of action that may arise from my child’s participation in the Program, including claims for bodily injury, personal injury, property damage, or wrongful death, whether caused by negligence or otherwise.

  1. Photo/Video Release

I grant LingoStars permission to use photographs and video recordings of my child participating in the Program for promotional, educational, or program-related purposes, both in print and online. If I do not consent to this, I will provide written notice to LingoStars in advance.

  1. Behavioral Expectations and Discipline

I understand that LingoStars has rules and policies for appropriate behavior during the Program. I agree to discuss these expectations with my child and understand that disruptive behavior may result in my child's removal from the Program without refund.

  1. Personal Belongings

I understand that LingoStars is not responsible for lost, stolen, or damaged personal belongings during the Program.

  1. Acknowledgment of Policies

I acknowledge that I have read and understood the Program’s policies regarding attendance, cancellations, and behavior, and I agree to abide by these policies.

  1. Indemnification

I agree to indemnify and hold harmless LingoStars from any claims, losses, liabilities, damages, costs, or expenses (including reasonable attorney’s fees) arising from my child’s participation in the Program.
